The direct deposit form is available for members who wish to have their Per Capita Payment and GTC Stipends deposited into their checking or savings account.
Please note that you are still required to complete, notarize and return a payment form to claim your per capita payment every year.
This request will remain in effect until the Enrollment Department receives written notice from the member to cancel the request.
