2014 HEALTH CARE CHANGES

ALL EMPLOYEE HEALTH PLAN COVERED EMPLOYEES WHO ARE ELIGIBLE TO PARTICIPATE IN REASONABLE ALTERNATIVE STANDARDS EVENTS 

All employees covered under the Oneida Health Plan may participate in “Reasonable Alternative Standards” for the full health plan incentive 10% weekly health insurance contribution rate: 

*Pulse Communication with participation information 

*List of activities you may earn RAS points in 

*Calendar of Events of specific RAS activities including event, provider, date, time, location, RAS points available 

Note that RAS participation is voluntary and must be done on your own time.

Wellness Incentive FormClick here to access the the dual-purpose claim form to claim your Wellness Incentive. The dual-purpose form may be used more than once to claim a specific incentive or reimbursement/incentive.  All completed forms must be submitted to Christina Bluebird at Human Resources.  Incentive payments will be processed though regular payroll.
