2nd and 4th Tuesday of each month from 1:00 PM - 3:00 PM

1st and 3rd Friday of each month from 8:30am - 10:30am

SERVICE DESCRIPTION:
Public Health service provides immunizations to individuals by appointments only.

ELIGIBILITY:
Member of a Federally recognized Tribe or Alaska Native Group who is an established patient of the Oneida Community Health Center.

COST:
No cost to the patient.

WHAT TO EXPECT:
1. For individuals less than 18 years of age, a parent or guardian must be present. If the parent or guardian is unable to attend, verbal or written permission must be presented before the child will be vaccinated.
2. Please bring a copy of your child’s immunization record with you, especially when receiving shots from other medical providers.
3. Education will be provided on recommended immunizations and when to return for further immunizations.
