Eligibility

Established with a Oneida Community Health Center Physician

Patients must be current patients at the Oneida Community Health Center meaning they must have been seen at OCHC within the last three years.

Available after hours to help determine the need for home care or urgent care.

 

They DO NOT determine Contract Health eligibility. Staff are on call at home and do not have access to computer information and/or the medical record.

Staff and Hours Monday-Friday:

1-Pediactrics Triage Line, available from 8:00A.M.-4:00P.M. staffed by two rotating Registered Nurses working under two Pediatricians and one Family Practice Physician.

1-OB/GYN Triage Line, available from 8:00A.M.-4:00P.M. staffed by three rotating Registered Nurses working under four OB-GYN Physicians, one Woman’s Health Nurse Practitioner and two Midwives.

2- Adult Triage Lines, available from 7:00A.M.-5:00P.M. staffed by five rotating Registered Nurses, 2 lines are open at a time. This staff works under OCHC physicians.

1-On Call Nurse Line, OCHC Registered Nurses are on-call all hours that OCHC is closed and can be contacted by calling the main OCHC line. One on-call line is open at a time. These staff members work up to 40 hours per week at the clinic and are on-call in addition to that time. This staff is available after hours to help determine the need for home care or urgent care.
