SERVICE DESCRIPTION:

Behavioral Health Social Workers are credentialed as Master Social Workers. The Targeted Case Management is a State and Federal Government Medicaid program. This is not a entitlement program. There are specific requirements to qualify for the program. Behavioral Health Social Workers receive referrals internally for patients already receiving Behavioral Health Services. The Targeted Case Management services assist clients and when appropriate, their families gain access to and coordinate an array of needed services. This includes medical, social, educational, vocational and other services pertaining to the clients needs. The three components of case management services are: Assessment, Case Plan Development and Ongoing monitoring and service coordination.

ELIGIBILITY:

Enrolled Oneida Tribal Members, Enrolled Native Americans from a Federally recognized tribe that resides in Brown or Outagamie County.Non-native or not insured spouses are eligible for service, however they will be billed for costs not covered, through third party billing.

 

COST:

Funding through Indian Health Services/Oneida Tribal Contributions, Medical Assistance/Medicare, Private Insurance, Private Pay.
