Descendant Application

Descendant Applications are for individuals who are not eligible for enrollment with the Oneida Tribe of Indians of Wisconsin. To be recognized as a Descendant you must establish ancestry to an enrolled member listed on the 1935 base roll. Descendant Certifications are used for assistance at the Oneida Health Center and/or other programs.

It is your responsibility to complete the form and to provide the necessary documentation to establish ancestry (see form).
